Obtaining a comprehensive patient history can prove to be challenging when patient records are inaccessible or incomplete, and patients are unable to recall or provide key information. It is not uncommon for pharmacists to approach other healthcare providers, access health records or contact the patient’s family member/carer for further clarification.

This CPD accredited course discusses how digital health tools, such as My Health Record and the my health app, can be utilised to support pharmacists in conducting medication reviews.

The course is divided into four topics: 

  1. Pharmacist medication reviews (10 m) 
  2. My Health Record & Medication Reviews (20 m)
  3. The my health app as a tool to empower patients (15 m)
  4. Benefits of digital health tools to support pharmacist medication review processes (10 m)

The course content and assessment should take approximately 60 minutes to complete.

This course is designed to provide you with a brief overview of My Health Record and how it can be used to support your decision-making and administration of the COVID-19 vaccine and other immunisations.

At the end of this module you will be able to:  

  • identify the relevant My Health Record documents that can be used to support the COVID-19 vaccination rollout
  • describe how each of these documents can be accessed and used in the My Health Record system
  • demonstrate how to access these documents within your clinical information system (CIS) or the web-based National Provider Portal (NPP).

It will take approximately 30 minutes to complete. You can leave the module at any time and return later.
