Advance Care Planning and My Health Record for Carers

Following National Advance Care Planning Week, join our webinar and questions and answers (Q&A) session to learn how to support your loved one to prepare for future health care decisions. Learn how you can upload advance care planning documents to My Health Record or get support to upload. This will enable documents to be accessed when they’re needed most.

A collaboration between Carers Australia, Advance Care Planning Australia and Australian Digital Health Agency, the webinar will feature an expert panel including Advance Care Planning Australia Community Ambassador and carer Jenny Dexter along with Carers Australia National Policy & Strategy Director Kelly Gourlay, Advance Care Planning Australia National Manager Samantha McCLoy and Australian Digital Health Agency Education Manager Kate Ellis.

Join us for this one hour webinar and 15 minutes Q&A to learn more about the roles of carers, support people and appointed substitute decision-makers in advance care planning.

Understanding your heart health with the Heart Foundation and My Health Record 

Join us this Heart Week (1-7 May) for an informative webinar featuring speakers from the Heart Foundation and the Australian Digital Health Agency. This session will equip you with the knowledge to better understand your personal risk of heart disease and manage your heart health information with My Health Record. You will learn the three simple steps that you can take to keep your heart healthy, as well as how and where you can access Heart Health Checks with your healthcare provider. The webinar will also cover topics such as how to access your My Health Record and my health app, including medicines information and heart-related test results such as pathology and INR reports. You will have an opportunity to have your questions answered by the speakers during the Q&A session at the end of the webinar.
